The case manager assesses participant needs, develops re-housing plans, and coordinates services to help people experiencing homelessness return to stable housing. As a team leader, the role also supports the Re-Housing Program Manager with administrative duties, staff development, and program oversight.

What you'll do

• Assess participant needs and develop re-housing plans
• collaborate with community partners using a Housing First model
• provide support, crisis intervention, mediation, and service coordination
• complete intake and case documentation
• maintain case records and program data
• support program rules
• assist with housing-related transportation when appropriate
• conduct community outreach and assessment as directed
• assist with staff training
• support the program manager with reports and attendance records.
