What you'll do

• Conduct intake and benefit assessments
• develop case plans and goals
• monitor progress
• provide advocacy and referrals
• meet with clients at their residences using a trauma-informed approach
• connect families with community resources and follow up on referrals
• document client contacts and outcomes in HMIS and Salesforce
• transport clients as needed in an agency vehicle
• participate in staff meetings, training, and supervision.
