What you'll do
- Coordinate and co-manage patient care and provide feedback to primary care providers
- provide on-site crisis triage and intervention as needed
- deliver brief, solution-focused behavioral health assessments and interventions
- consult with medical providers and care team members
- provide assessment, diagnosis, treatment, referrals, and follow-up
- maintain EHR documentation
- comply with HIPAA, OSHA, and organizational requirements
- participate in staff meetings and quality improvement
- collaborate with community organizations.
