Safe Ride Case Manager

What you'll do

• Coordinate Safe Ride referrals and warm handoffs
• complete intake and assessments
• provide advocacy, crisis intervention, counseling, and case management
• develop service and housing plans with participants
• connect clients with housing and community resources
• manage inflow and outflow for six CTA beds
• document services in HMIS and Salesforce
• communicate client progress and bed availability to partners.
