• Provide targeted case management under program and Medicaid requirements
• assess needs and develop, implement, and monitor service plans
• coordinate referrals to healthcare, behavioral health, education, and community resources
• advocate for clients
• maintain contacts and accurate records
• collaborate with families, schools, agencies, and providers
• participate in case staffing and quality assurance.
