Job overview

The Community Health Worker — Health Steps Grant role is onsite in Lynn, MA, with a Monday–Friday schedule from 8:00 a.m. to 4:00 p.m. Compensation is $19.76–$29.29 per hour. The role supports Lynn youth and works with the nurse practitioner, behavioral health provider, and other Lynn Community Health Center team members. The worker helps youth and families address medical, social, and psychological needs by providing education, advocacy, coaching, care navigation, and outreach. The role supports access to ongoing primary and behavioral health care and continuity of care.

What you'll do

• Educate, advocate for, and coach Lynn youth
• help patients and families navigate health care and social service systems
• arrange transportation and accompany patients to appointments as requested by the care team
• participate in community outreach
• conduct engagement activities and re-engage patients or families who miss appointments or do not follow up on treatment.
