The Healthcare Coordinator role is onsite in Tomahawk, WI. Compensation is not specified; the posting offers a $10,000 sign-on bonus paid in installments at 6, 12, 18, and 24 months of continued employment, subject to eligibility requirements. The role partners with the Community Director in a senior living environment focused on resident care and support. The Registered Nurse leads resident care programs and develops the care team, helping provide individualized, compliant, and people-centered support. The role coordinates care, medication management, staff training, and department operations while responding to resident emergencies and community needs through 24/7 on-call support.

What you'll do

• Assess residents’ needs and create individualized service plans
• coordinate resident care and EHR/eMAR documentation
• monitor medication management and conduct reviews
• communicate with physicians, families, and staff about health concerns
• recruit, hire, train, supervise, and evaluate resident assistants and healthcare staff
• lead onboarding and ongoing training
• ensure regulatory compliance and safety
• review and follow up on incident reports
• manage department budgets, staffing schedules, and medical inventory
• provide 24/7 on-call support for resident emergencies and community needs.

Why this role
