Case Manager – Children’s Residential Habilitation

Job overview

The Case Manager – Children’s Residential Habilitation role is onsite in Idaho Falls, ID, with compensation of $18–$20 per hour. The case manager supports youth residing in Riverside Academy facilities and reports to the Program Director.

What you'll do

• Provide case management for resident youth
• conduct monthly meetings with each youth and assigned staff
• facilitate intake and exit processes
• coordinate with youth, families, and team members
• coordinate and lead planned youth activities
• attend appointments
• train staff on new youth’s care
• maintain confidentiality and teach privacy requirements
• and follow current policies, emergency procedures, and safety protocols.
