What you'll do

• Serve as the primary Hub spokesperson
• convene partners to assess needs and set priorities
• manage localized program implementation and coordinate statewide initiative rollouts
• monitor progress and escalate risks
• oversee RHTP Navigator utilization and workflows
• facilitate Leadership Board governance
• coordinate partner onboarding
• support referral pathways and operational workflows
• track and report Hub data using dashboards and Community Information Exchange systems
• identify barriers and coordinate improvement activities
• maintain local stakeholder presence and support implementation readiness.

Monday–Friday, 8:00 a.m.–5:00 p.m.; times may vary, and occasional evening or weekend outreach may be required. Frequent travel throughout assigned Hub 15 geography. Associate degree or equivalent combination of education and relevant experience; lack of post-secondary education alone will not disqualify applicants.
