Description

Provide skilled nursing interventions in the treatment of patient's illness, rehabilitative needs and preventative care, under the direction of a Registered Nurse. Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Maintain contact with patients, physicians, and other members of the healthcare team. Participate in care coordination and discharge planning.
