Resource Coordinator

The full-time Resource Coordinator role is onsite in Philadelphia, PA. Pay is $23.85 per hour. The position supports FaSST/Connections participants experiencing homelessness and facing mental health challenges or substance use, through advocacy, ongoing support, and empowerment. The coordinator works with participants and their support systems to address barriers and pursue individual goals, including access to treatment, income, legal support, and education. The role also connects participants with services, monitors service delivery, and collaborates with outreach teams and community agencies.

What you'll do

• Assess participant needs and connect them to appropriate services
• maintain regular contact with participants and service providers
• develop and review individualized strength-based goal plans
• conduct home visits and accompany participants to appointments as appropriate
• advocate for benefits and entitlements
• coordinate outreach activities
• maintain service records and submit reports.
