What you'll do

• Assess veterans’ needs and develop individualized service plans
• monitor progress and adjust plans
• document interactions and plans in EHR systems
• coordinate community resources and providers
• conduct visits and check-ins
• advocate for healthcare, employment, and benefits access
• maintain confidential case files and reports
• facilitate life-skills, employment-readiness, and financial-literacy sessions
• collaborate with multidisciplinary staff
• provide crisis intervention and de-escalation
• support housing retention, eviction prevention, and conflict resolution
• ensure program compliance using strengths-based and trauma-informed approaches.
