• Monitor referral platforms and other referral sources
• review referrals for completeness, accuracy, and eligibility
• contact families and referral partners for missing information
• enter, process, track, and assign referrals
• maintain records and reporting data
• assist with referral analysis and quality improvement
• conduct assigned family re-engagement outreach
• and connect ineligible families with community resources and relevant MECA programs.
