What you'll do

• Coordinate with primary care physicians, pediatricians, psychiatrists, and other providers
• conduct nursing assessments and develop nursing care plans
• recommend interventions and provide preventive-care education
• engage members or caretakers on alternatives to emergency or hospital care
• participate in hospital discharge processes
• reconcile medications and support medication adherence, including administering oral and intramuscular medications as needed
• facilitate laboratory testing and medical histories
• maintain records
• attend consultation, supervision, and training
• follow confidentiality and regulatory requirements.
