What you'll do
- Build relationships grounded in shared experience
- develop and update member wellness plans
- support motivation, skill-building, and readiness for change
- provide education on harm reduction and recovery options
- connect members with peer groups and community resources
- help address barriers such as housing, transportation, benefits, and service access
- coordinate with providers and community partners
- support discharge planning
- document contacts and plan updates
- protect confidentiality
- participate in supervision, team meetings, and training.
