Hospice Admissions Registered Nurse, Weekends

The role supports CorsoCare Hospice admissions and patient and family care. The nurse assesses patients referred for hospice, coordinates admission and care planning, and educates patients and families about hospice services and end-of-life options. The role also provides symptom support, documentation, and collaboration with an interdisciplinary care team.
