What you'll do

• Conduct chart reviews and assess biochemical and anthropometric measurements, dietary history, and medical history
• develop, implement, and follow up on individualized nutrition care plans
• provide nutrition counseling and medical nutrition therapy to adult and prenatal patients
• educate prenatal clients on weight gain, supplementation, and substance avoidance
• document care in electronic medical records
• provide evidence-based and disease-specific nutrition education
• collaborate with providers, pharmacists, nurses, social workers, and other staff
• develop patient education materials.
