What you'll do

• Conduct hospice consultation and admission visits
• assess patient and family needs and clinical eligibility with attending and hospice physicians
• complete admission assessments and election-of-benefit documentation
• develop and update plans of care with the interdisciplinary group
• coordinate medications and durable medical equipment for same-day delivery
• participate in after-hours and weekend on-call rotation.
