What you'll do

• Coordinate clinical discharge planning and monitor progress toward clinical and discharge milestones. Collaborate with emergency department and admitting providers, utilization management, clinical documentation integrity, social work, and case management support. Identify and escalate barriers to discharge and throughput
• facilitate interdisciplinary rounds and progression huddles. Complete assessments and timely documentation, arrange post-acute referrals, and partner with patients and families on transition plans.
