Case Manager — Substance Use Recovery

The Case Manager — Substance Use Recovery role is onsite in Laguna Niguel, CA, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. Compensation is not specified. The role supports clients in the Laguna Coast Recovery program. The case manager coordinates assessments, treatment planning, counseling, and recovery services to help clients participate in treatment and prepare for discharge. The role also connects clients with supportive people and resources and works with the clinical team on care needs and crises.

What you'll do

• Orient new clients to program expectations and services
• help clients set and pursue goals
• coordinate with family, friends, partners, and outside resources
• provide behavioral health education and daily skills training
• collaborate with the clinical team and assist with crisis intervention
• support aftercare planning
• maintain client confidentiality.
