What you'll do

• Assess clients and families to determine home care needs
• reevaluate nursing needs
• initiate and revise plans of care
• develop nursing diagnoses and care plans with client and family input
• provide prescribed medications, treatments, and direct nursing care
• counsel and instruct clients and families
• identify and implement discharge planning needs
• coordinate assigned caseloads
• prepare clinical notes and communicate with physicians and community health partners
• complete documentation by 10 a.m. the next day
• supervise and evaluate home health aide care every two weeks
• manage client supplies and appointments
• organize travel and document mileage
• participate in event management and safety processes.

Why this role

Schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with rotation every sixth weekend, two holidays per year, and on-call as needed. Requires a current Minnesota nursing license and graduation from an accredited nursing school; two years of RN experience is required, with home care or hospice experience preferred. Mileage reimbursement is 72.5 cents per mile.
