Hospice and Home Health Registered Nurse (RN) Case Manager – Per Diem is an onsite role based in San Bernardino, CA, serving patients in their residences or residential facilities throughout Los Angeles County. Pay is $75–$100 per hour. The role supports Angel Care Hospice and Palliative’s end-of-life care and family support services. The RN creates and manages individualized patient care plans, coordinates with the interdisciplinary hospice team, administers medications, monitors for reactions, and documents and reports changes in patient condition. The role contributes to effective, compassionate care through accurate records, timely communication, and adherence to professional standards.
