The role contributes to safe, coordinated support for consumers in the program.
What you'll do
- Provide direct care including bathing, dressing, toileting, and medication assistance
- document progress, activities, tasks, and responses to care
- report condition changes to the RN and Program Supervisor
- provide guidance and assist with medication management under RN, LPN, and pharmacist direction
- follow agency policies and applicable regulations
- transport consumers as necessary.

Why this role
Current Certified Nursing Assistant licensure and a valid driver’s license with acceptable DMV record are required. One year of nursing experience and long-term care experience are preferred.
