Service Coordinator, Human Services

Job overview

The Service Coordinator role is onsite in Iowa City, IA, and compensation is $24 per hour. The coordinator is the contact point for services provided by Reach For Your Potential and works with members, families, guardians, providers, and staff. The role coordinates individualized services and supervises staff to help ensure members receive planned supports, maintain well-supported living arrangements, and access appropriate services through the organization and outside agencies.

What you'll do

• Assist with hiring, promotion, supervision, training, evaluation, discipline, and termination of staff
• coordinate individual service planning and semiannual reviews
• work with interdisciplinary teams to identify services
• maintain contact with members, families, guardians, and agencies
• coordinate transportation and member transitions
• visit member locations and ensure homes are maintained
• support scheduling, documentation, and interdepartmental coordination
• respond to concerns within 48 hours.
