What you'll do

• For RNs, assess physical, psychosocial, and spiritual needs
• for LPNs, gather related patient and family data
• provide intermittent skilled nursing under physician direction
• monitor pain and symptoms and assist patients and families
• liaise with physicians
• provide emotional support and care instruction
• refer patients and families to services
• visit the residence at time of death
• complete documentation
• relay information to team members
• participate in team meetings and training
• RNs participate in triage coverage as needed.
