What you'll do

• Assess patients’ and caregivers’ social, spiritual, and emotional needs
• participate in interdisciplinary care planning and review
• help patients and families prepare for illness-related changes and advance care planning
• connect them with hospice and community resources
• support bereavement services
• and provide discharge planning for changes in level or location of care.
