The Care Professional — Home Care role is onsite in Hilton Head Island, South Carolina. Compensation is $16–$30 per hour. SYNERGY HomeCare of Lowcountry provides personalized home care and support to clients in the Hilton Head Island community. The role assists clients with personal care and daily living, supports independence, and helps maintain a safe and supportive environment. It also contributes to coordinated care through communication with clients, families, caregivers, and the care team.

What you'll do

• Assist clients with bathing, dressing, grooming, mobility, and toileting
• help with meal preparation, medication management, and vital-sign monitoring
• maintain a safe environment in accordance with company policies and procedures
• communicate with clients, families, caregivers, and the care team, including daily reports and attendance tracking.
