What you'll do

• Provide intake, screening, assessment, case planning, counseling, crisis intervention, and case management
• monitor client services and advocate for assigned clients
• maintain confidential case records, service plans, reports, and required documentation
• enter and retrieve client data in DuPage County’s HMIS
• connect clients with community resources and service providers
• work with clients and housing project leadership on family housing needs.
