The Social Worker role is onsite at the Lisbon Avenue Health Center in Milwaukee, WI, with a Monday–Friday schedule from 8 a.m. to 5 p.m. Compensation is not specified. What you'll do

• Collaborate with providers to identify patients needing social work services
• conduct initial and periodic assessments and prioritize follow-up
• develop and evaluate care plans with patients, families, and the health care team
• identify and use community resources
• assist with transportation, scheduling, housing, and food resources
• support patient self-management and support systems
• maintain timely documentation
• participate in committees
• contribute to social work program development.

Why this role

A master’s degree in social work is preferred, and applicants with a bachelor’s degree in social work may apply. Wisconsin Social Worker licensure is required; master’s-level applicants may obtain APSW within the first six months.
