What you'll do

• Build relationships with hospital case managers, discharge planners, social workers, physicians, and community referral partners
• generate skilled nursing and rehabilitation referrals
• visit hospitals and referral partners
• develop and execute a market sales plan
• partner with admissions and clinical teams on patient transitions
• track referral activity, admission conversions, and growth opportunities.
