The therapist provides behavioral health care to residents through assessments, individual and group therapy, and behavior planning. Recommendations to facility staff and clinical documentation support residents’ care in their assigned territory.

What you'll do

• Complete initial psychosocial assessments
• provide individual and group therapy
• develop resident behavior plans
• recommend behavioral and therapeutic interventions to nursing home staff
• enter patient information and clinical documentation in the EMR
• travel daily to skilled nursing facilities.
