What you'll do

• Supervise and engage assigned clients according to care plans
• facilitate activities and outings aligned with support plans
• provide transportation as specified
• assist with activities of daily living
• complete training and client-care documentation
• communicate changes to leadership and care teams
• travel up to 50 miles for shifts as needed.
