What you'll do

• Travel to patient homes and community settings in Chenango County and assigned service areas
• conduct comprehensive nursing assessments and head-to-toe evaluations, including medication and diagnosis reviews
• develop, review, and update Plans of Care
• supervise and orient home health aides
• provide clinical guidance to aides and caregivers
• monitor changes in patient condition and communicate concerns
• collaborate with patients, families, caregivers, aides, and office staff
• complete timely clinical documentation
• follow healthcare regulations, clinical standards, and company policies.
