The caregiver provides one-on-one support that helps clients remain in their homes, assists with daily routines and mobility, and keeps families and the care coordination team informed about client needs and condition changes.

What you'll do

• Assist clients with bathing, dressing, grooming, oral care, and hygiene
• support safe transfers and mobility
• prepare meals and provide light housekeeping
• offer companionship
• observe and report changes in condition or behavior to the care coordination team
• run local errands, assist with grocery shopping, and provide transportation to appointments or community outings.
