What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to other disciplines as indicated
• teach self-care and support patient independence
• supervise LPNs and HHAs
• complete timely, accurate documentation
• communicate patient progress
• plan and provide staff education
• perform assigned on-call responsibilities and services
• participate in clinical development and continuing education.
