Health Enrollment Navigator (Bilingual English/Spanish)

Job overview

The Health Enrollment Navigator role is onsite in San Francisco, CA, with a starting hourly range of $28.01–$30.11 and a Monday–Friday schedule from 10:00 a.m. to 7:00 p.m., with availability required every other Saturday. The role reports to the Patient Services Manager at Mission Neighborhood Health Center, a community health organization serving San Francisco’s Mission and Excelsior districts. The navigator connects potentially eligible residents with Medi-Cal enrollment and renewal support. Through outreach, education, application assistance, and community partnerships, the role helps vulnerable populations access health coverage and establish a health home.

What you'll do

• Develop and implement community engagement plans
• lead public education at clinic sites
• create Spanish- and English-language outreach materials
• coordinate workshops, clinics, and community meetings
• conduct weekly outreach at schools, events, and neighborhood corridors
• assist with Medi-Cal applications and renewals
• maintain participant records and reports
• protect confidential health information under HIPAA.
