What you'll do

• Complete evaluations and individualized plans of care
• provide skilled treatment for communication, cognition, and swallowing needs
• educate residents, families, caregivers, and team members
• collaborate with nursing, wellness, and rehabilitation staff
• identify changes that may warrant intervention
• document clinical care.
