What you'll do

• Facilitate multidisciplinary care for an assigned patient panel
• review medical records to diagnose and treat conditions
• provide care management and education to patients and families
• coordinate referrals to healthcare providers and specialists
• evaluate and manage acute and chronic conditions with multiple comorbidities
• collaborate with physicians, residents, medical students, advanced practice professionals, staff, and support personnel
• contribute to academic, educational, and research initiatives
• serve as a community resource.
