The Registered Dietitian Nutritionist (RDN) — Diabetes Care & Education Specialist role is onsite in Rock Hill, SC, with a salary range of $60,000–$75,000 per year. Affinity Health Center is a Federally Qualified Health Center serving patients across York County and is building a Chronic Disease Management Program. The RDN provides nutrition and diabetes education expertise within an interdisciplinary care team, helping patients with type 2 diabetes and prediabetes manage their health. The role also contributes to developing and improving the program, including education materials, curricula, and outcome tracking.

What you'll do

• Provide individualized medical nutrition therapy for type 2 diabetes and prediabetes
• deliver individual and group diabetes self-management education and support
• educate patients about continuous glucose monitoring and collaborate with providers on glucose trends
• create culturally responsive meal plans
• partner with community health workers and peer educators
• contribute to program design, curricula, education materials, and outcome tracking.
