What you'll do

• Provide direct services including engagement, intake, assessment, service plan development, referral, advocacy, therapeutic counseling, reassessment, and exit summaries.
• Manage a caseload, monitor client progress, provide group counseling, coordinate services with mental health and medical providers, conduct home and field visits, and complete HRA housing applications.
