The Clinical Nurse (RN) role is onsite in Silver Spring, MD. Compensation is $70,000–$81,000 per year. The nurse is part of Community Clinic Inc.’s integrated care team, supporting primary care and coordinated patient care. The RN provides direct nursing care, care coordination, and patient education to help improve outcomes across patients from infancy through geriatrics. The role works with clinicians and other care team members on assessments, treatment plans, referrals, follow-up, and quality improvement.

What you'll do

• Provide routine nursing care and care coordination
• participate in clinical huddles and pre-visit planning
• assess, screen, and triage patients
• provide medical advice and discharge instructions
• reconcile medical, vaccine, and medication records
• coordinate referrals and transitions of care
• educate patients on chronic disease management and prevention
• administer treatments, vaccines, and medications
• support quality improvement, staff training, and electronic medical record documentation.
