Hospice Chaplain

What you'll do

• Assess spiritual needs and ensure timely services
• participate in comprehensive assessments and individualized care planning
• provide direct spiritual care
• liaise with community chaplains and spiritual counselors
• document services and communications
• attend patient care conferences and interdisciplinary meetings
• provide spiritual-care consultation and education
• recruit community spiritual-care resources and volunteers
• participate in quality improvement activities
• provide funeral or memorial services when requested and plan periodic memorial services.
