What you'll do

• Manage a caseload of around 45 individuals
• match clients with direct care staff
• review, implement, and monitor Individualized Support Plans under Pennsylvania ODP waiver guidelines
• submit case evaluations and compliance reports to the Towanda office
• coordinate with county entities, medical professionals, and families
• conduct field visits
• maintain electronic case records, progress notes, and service authorizations
• travel across the listed counties.
