Medical Intake Coordinator

The role works across insurance, scheduling, pharmacy, and clinical teams to support smooth referral processing and prevent delays in care.

What you'll do

• Document accurately and promptly in the EMR
• coordinate patient care with home health, pharmacy, and insurance teams
• obtain required referral documentation from medical providers
• maintain knowledge of reimbursement policies
• communicate referral status and updates to patients
• enter referral information
• complete follow-up tasks
• help prevent delays in patient care
• attend meetings and in-services as requested
• assist with new staff orientation and training.

Why this role
