What you'll do

• Complete comprehensive psychosocial assessments within five days of hospice election
• identify patient and family needs
• assist with advance directives, funeral and discharge planning, and family counseling
• provide end-of-life and grief counseling
• attend interdisciplinary team meetings and coordinate care
• make community resource referrals.

Why this role
