Addiction Engagement Counselor

The Addiction Engagement Counselor role is onsite in Vancouver, WA; compensation is $86,403–$115,002 per year. It contributes to coordinated treatment through assessment, service planning, referrals, medication-related care coordination, and collaboration with a multidisciplinary team.

What you'll do

• Provide individual, group, and family clinical services through screening, assessment, diagnosis, and service planning
• provide case management, triage, referrals, and coordination with providers, families, and outside agencies
• coordinate care for members receiving medication from the Addiction Medicine Medical Team
• document care in the electronic medical record
• contribute to program development, in-service training, and process improvement
• provide face-to-face and virtual care.
