What you'll do

• Assess patients and collaborate with the care team to develop and implement individualized plans of care.

• Maintain accurate and timely progress records, document pain and symptom management changes and outcomes, teach patients, families and caregivers care techniques, and communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and services as assigned.
