What you'll do

• Manage medical records and the information system
• maintain complete case records
• support quality improvement, utilization review, and performance improvement through data collection and documentation
• plan and coordinate office and community services
• review clinical documents and service plans
• facilitate data sharing with recipients
• provide face-to-face, telehealth, and collateral services.
