Performs initial, ongoing, and functional health status assessment (30%). Documents planned nursing interventions to achieve outcomes appropriate to patient needs (30%). Evaluates and documents response to nursing interventions and achievement of outcomes; revises plan of care based on evaluative data (20%). Actively participates in process improvement activities (10%).
