The Dietitian role is onsite in Richmond, VA; compensation is not specified. The position supports InnovAge PACE participants through nutrition care and related services. The Dietitian assesses participants’ nutritional status and develops and implements nutrition care plans to support their health needs. The role also provides nutrition education, coordinates with the interdisciplinary team and caregivers, and contributes to meal-service administration and quality improvement for nutrition services.

What you'll do

• Complete nutritional assessments and reassessments
• develop and implement nutrition plans of care
• assess food access and refer needs to the interdisciplinary team
• determine and order nutritional supplements
• provide dietary education to participants, caregivers, and staff
• monitor food intake and therapeutic diets
• document nutrition notes
• support meal service, menu review, sanitation checks, care planning, and nutrition quality improvement.
